Add SDK for programmatic AgentSession usage

- Add src/core/sdk.ts with createAgentSession() factory and discovery functions
- Update loaders to accept cwd/agentDir parameters (skills, hooks, custom-tools, slash-commands, system-prompt)
- Export SDK from package index

Addresses #272
