Add extensions option to createAgentSession SDK

Browse source 
- Accept ExtensionFactory[] for inline extensions (merged with discovery)
- Mark preloadedExtensions as @internal (CLI implementation detail)
- Update sdk.md with inline extension example
- Update CHANGELOG
