Fix Vercel AI SDK v6 protocol compliance

Browse source 
- Add messageId to start chunk (required by useChat)
- Remove undocumented tool-output-error wire type, use
  tool-output-available for all tool results
- Pass structured tool output through instead of JSON-stringifying
