Add permission request handling to the Inspector UI so users can Allow, Always Allow, or Reject tool calls that require permissions instead of having them auto-cancelled. Wires up SDK onPermissionRequest/respondPermission through App → ChatPanel → ChatMessages with proper toolCallId-to-pendingId mapping.
